This was sent to me by a friend the other day. I would be laughing hysterically if I was the nurse who received this patient form but I hear she was less than amused. Very clever indeed. When asked what he spent it on, the reply was…”an eye exam.” The best thing about this? I’m guessing that next time you go to the doctors office each and every one of you is going to write something similar if you get the chance.
